Place potatoes and in bottom of crock pot. Sprinkle with salt and pepper, to your taste preference.
Place chicken and carrots on top of potatoes.
In a small bowl, whisk together soup and chicken broth until smooth. Pour over chicken/carrots/potatoes.
Cover and cook on low for 5-6 hours.
Remove chicken, potatoes and carrots with a slotted spoon (leaving liquid in pot) and place in a serving dish.
Turn crock pot on high and stir in cheddar cheese and Worcestershire sauce. Cook until cheese is completely melted and mixture is smooth ( about 15-30 minutes)
Spoon cheese sauce over chicken and potatoes before serving.
